Veritas Policies & Compliance
Booking Policy

Appointments may be scheduled by phone, online, or in person.

A valid phone number and/or email address is required to secure an appointment.

New patients may be asked to complete intake forms prior to their visit; completing these ahead of time helps ensure timely care.

Certain appointment types may require a deposit or pre‑authorization depending on the service.

Patients are encouraged to arrive 10–15 minutes early to complete any remaining paperwork.
Late Arrival Policy

To respect the time of all patients, appointments begin and end at the scheduled time.

Patients arriving more than 10 minutes late may experience a shortened visit or may need to be rescheduled, depending on provider availability.

If a late arrival results in a rescheduled appointment, it may be treated as a late cancellation according to the cancellation policy.

Repeated late arrivals may require prepayment for future appointments.
Cancellation Policy

We kindly request at least 24 hours’ notice for appointment cancellations or changes.

Cancellations made with less than 24 hours’ notice may be subject to a cancellation fee.

Fees must be paid before scheduling future appointments.

Patients with repeated last‑minute cancellations may be required to prepay for future visits or may be limited in scheduling options.
No‑Show Policy

A “no‑show” occurs when a patient does not arrive for a scheduled appointment and does not contact the clinic beforehand.

24-hour notice is required for cancellation of routine visits

48-hour notice is required for procedures and extended visits

No-show fee: $85–$100 depending on visit type

Repeated missed appointments may result in dismissal from the practice

No‑shows may incur a no‑show fee, which must be paid before future appointments can be booked.

No-show fees are not billable to insurance and must be paid prior to future scheduling.

Exceptions may be granted in the case of documented emergencies.

Multiple no‑shows may result in limited scheduling privileges or dismissal from the practice, depending on severity and frequency.

In the event a patient has incurred three (3) documented “no-shows” and/or “same-day cancellations,” the patient may be subject to dismissal and will no longer be able to utilize our services.
No Refund Policy

Payments made for medical services, membership fees, procedures, or administrative fees are non‑refundable.
-
This includes prepaid services, deposits, and membership dues, regardless of usage.
-
If a service must be discontinued due to a clinical decision, the clinic may offer credit toward another service at its discretion.
-
Membership fees are not prorated for partial months or unused services.
Emergency & Special Circumstances Policy

We understand that emergencies and unforeseen events happen. If a patient experiences a medical emergency, family emergency, or other unavoidable circumstance, cancellation fees may be waived at the clinic’s discretion.

Patients should notify the clinic as soon as reasonably possible if an emergency prevents them from attending their appointment.

If a patient experiences a medical emergency while at the clinic, staff will follow established emergency protocols, including contacting emergency services if needed.

In the event of a clinic‑wide emergency (power outage, provider illness, natural disaster, etc.), the clinic will notify patients promptly and assist with rescheduling.
Direct Primary Care (DPC) Disclaimer
Veritas Healthcare PLLC offers a Direct Primary Care (DPC) membership model for eligible patients.
DPC is:

A voluntary membership-based healthcare model

Not health insurance
-
Not a replacement for major medical insurance

A direct financial agreement between patient and provider
DPC membership covers only the services explicitly listed in the membership agreement. DPC does NOT cover. Patients are responsible for:

Hospitalization
-
Emergency care
-
Specialist visits outside the practice
-
Imaging, labs, or medications not included in the membership
The clinic does not bill insurance for services provided under the DPC membership. Patients are strongly encouraged to maintain health insurance coverage for catastrophic events.
DPC agreements are month-to-month unless otherwise specified in writing.
Medicare Notice
Veritas Healthcare PLLC may offer services to Medicare beneficiaries under the Direct Primary Care model.

Veritas Healthcare PLLC does NOT bill Medicare for services covered under a DPC agreement.

Patients may not submit DPC membership fees to Medicare for reimbursement.

Medicare beneficiaries must sign a private contract acknowledging that services provided under DPC will not be billed to Medicare.

Medicare patients retain the right to seek Medicare-covered services outside of Veritas Healthcare PLLC.
If the provider has formally opted out of Medicare, this will be disclosed in writing and acknowledged by the patient prior to enrollment. Patients are encouraged to review Medicare rules and consult Medicare directly if they have questions regarding coverage.
